Initial commit: RAG MCP Server with relationship graph

Features:
- Vector search with Pinecone + Vertex AI embeddings
- Document relationships (link, unlink, related, graph)
- Auto-link with LLM analysis
- Intelligent merge with Gemini

Modular structure:

- tools/: core, relations, stats
